SPARCForms User Guide

As part of the SPARCRequest submission process, Form Functionality allows Service Providers to ask their customers service-specific questions that will help them to efficiently and effectively fulfill each request. It provides a flexible and powerful form creation tool and admin interface to empower Catalog managers and service providers to extend SPARC's data management to meet their unique needs.

How to Utilize Form Functionality as a SPARCRequest General User

Now that the Form Functionality has been built in SPARCCatalog at both the Service and Organization level, users are able to use the Form Functionality to provide additional details to service providers.

If Form Functionality exists on a requested service, the "Forms" section will appear on SPARCRequest Step 4 (Documents & Forms) Page.

 

 When the "Complete" option is selected, the form appears as shown below and must be completed.

 

When accessing the study in SPARCDashboard:

  1. Selected service requests that have forms still needing attention, the Forms column will show next to your request(s) in Red color.

 

  1. After a form is filled out, it is then viewable and/or editable from the SPARCDashboard "Form Submissions" table.

 

Responding to Surveys as a SPARCRequest General User

You may be asked to respond to two different types of surveys:

  1. A system satisfaction survey in SPARCRequest Step 5 (Review Your Request) when submitting service requests. This option will be a pop-up window when you click ‘Submit Request’

 

 

  1. A service satisfaction survey when a service request has been completed by the service provider. You will be emailed a request to fill out the survey.
