The Forms/Survey Responses table shows all forms/surveys that fit the filter criteria chosen on the left side panel. You can create filters and save them for future use.  From the Forms/Survey Responses table, providers can edit and preview form submissions as well.

The SCTR Satisfaction Survey is triggered and sent out once a request is completed (status changed to completed).

The Survey is sent to the PI and his/her research team once a service request has been completed. The PI and his/her team will receive an email notifying them that they can fill out the survey.

The Survey asks users how likely they are to recommend a particular SCTR service to colleagues and to provide any additional feedback. It also asks PIs and their research teams to cite SCTR-MUSC CTSA grant in any recent or future publication.
