Utilizing the SPARCForms page
Across the top of the screen, is the toolbar with the different SPARC modules. Click SPARCForms module to access all completed forms and surveys that you have access to. A list of rights for SPARCForms and Surveys can be found here.
The table of Form Responses shows all forms or surveys that fit the filter criteria. You can create filters and save them for future use. From the Form Responses table, you can edit and preview form and survey submissions as well.

As part of the SPARCRequest submission process, Form Functionality allows Service Providers to ask their customers service-specific questions that will help them to efficiently and effectively fulfill each request. It provides a flexible and powerful form creation tool and admin interface to empower Catalog managers and service providers to extend SPARC's data management to meet their unique needs.
Form Functionality as a SPARC User
Now that the Form Functionality has been built in SPARCCatalog at both the Service and Organization level, users are able to use the Form Functionality to provide additional details to service providers.
If Form Functionality exists on a requested service, the "Forms" section will appear below.
When the "Complete" option is selected, the form appears as shown below:
If selected services in SPARCDashboard have forms that still need attention, the Forms column will show next to your request in highlighted fashion.
After a form is filled out, it is then viewable and still editable from the SPARCDashboard "Form Submissions"
