LA-CATS will create a story in Pivotal Tracker to create demographics fields
Issue with making demographics a required field
When someone add an authorized user, they will be able to answer the demographics question for said user
Potential Solution: Remove “personal” demographics (race, sex, ethnicity, gender) from the add authorized user screen. The only place to see/edit this information is through the user profile page. This way we can keep demographics as a required field. Also, leaving those “personal” demographics on the add authorized user page makes them somewhat public.

There has to be a trigger for surveys to send.
There is a System Satisfaction Survey that can be setup to send each and every person after they submit a request
